Esomeprazole
About
Esomeprazole works by binding irreversibly to the H+/K+ ATPase in the proton pump. Because the proton pump is the final pathway for secretion of hydrochloric acid by the parietal cells in the stomach, its inhibition dramatically decreases the secretion of hydrochloric acid into the stomach and alters gastric pH .
Indication
Treatment of Heartburn , GERD , Gastritis & Ulcer
Side Effects
Headache , GI upset , Skin reactions , Dry mouth. Rarely angioedema . Rarely angiooedema , anaphylaxis , fundic gland polyps.
Drug Interactions
Phenytoin , warfarin , clopidogrel , ketoconazole , itraconazole , voriconazole , diazepam , citalopram , imipramine , clomipramine , CYP3A4 inhibitors or inducers , tacrolimus , digoxin , erlitinib , methotrexate
When not to Use
It is contraindicated in patients with known hypersensitivity to proton pump inhibitors (PPIs). Hypersensitivity reactions e.g. angioedema and anaphylaxis, bronchospasm, acute interstitial nephritis and urticaria have been reported with esomeprazole use.
Precaution
Daily long-term use (e.g. longer than 3 years) may lead to malabsorption or a deficiency of cyanocobalamin.
